No Insurance Required

Insurance is recommended, but not required.  There is no billing set up for Medicare, Medicaid (AR or OK) or any private insurance companies.

Patient Subscriber

A Patient Subscriber is a patient who signs the patient agreement, and pays a monthly fee to have Dr. Cook as their primary care physician.  Patient Subscribers have 24/7 phone/text access with Dr. Cook.

Medications

Oklahoma and Arkansas allow physicians with a dispensing license to dispense medications directly to patients.  Your medications will likely be in-hand when you leave the clinic.

What is a:

“Patient Subscriber”, or “Patient Subscription”?

A “Patient Subscriber”, is a patient who completes the Patient Agreement, and pays a registration fee and a monthly rate to have Dr. Cook as their Primary Care Physician.  No insurance needed.  A Patient Subscriber has Dr. Cook’s cell number and can call or text with urgent concerns - or call the Homestead DPC clinic number to arrange a scheduled visit for non-urgent matters.

How much does it cost?

$100/month.  No co-pays.  No “co-insurance”. 

New for 2024:

Ages 25 and under - $50/month

No long-term contracts.

What is included in the subscription?

Clinic visits, phone visits and electronic visits are included. Treatment for Hypothyroid, Hypertension (high blood pressure), Depression, Anxiety and other Psychiatric disorders, Diabetes, Rheumatology disorders, and other chronic diseases. In-office treatments such as joint injections, skin procedures like biopsies and cryo, minor injuries (sutures, boils, cysts, etc).  Dr. Cook can coordinate healthcare by phone, video calls or messaging in many cases, when appropriate.

Are all Medications available?

Most medications - blood pressure, cholesterol, antibiotics, etc. - provided at no additional charge

 

Dr. Cook avoids dispensing narcotics (Percocet, etc), sedatives/anxiolytics such as benzodiazepines (Xanax, Ativan, Valium, etc.), barbiturates, and/or other controlled substances regulated by the DEA and/or the OBNDD.  Newer brand name medications will be be available at cost plus a small additional charge to cover handling - if the medication is available for independent physicians.

If I have health insurance or Medicare/Medicaid, can I still be a member?

Call the clinic and leave a message to discuss your options.

What happens if I need to see a different specialist, such as a Cardiologist?

Dr. Cook has trained in most specialties, and medical conditions will be treated appropriately.  However, when a specialist's consultation is required, the patient subscriber will be responsible for payments to the specialist and/or the specialist's facility.

Homestead DPC will be ready to assume specialist, post-hospital, and post-ER follow-up care after discharge.

Do I need health insurance if I am a Patient Subscriber?

Health insurance is strongly recommended but not required.  Primary care covers many areas of medical need - but not all.  Insurance is recommended for Emergency Department, Hospital, and imaging or procedures outside the services provided by Homestead Direct Primary Care. Some examples: there is no Heart Catheterization lab in the clinic.  There is no operating room for emergency surgery.  A primary care facility has limitations for care.

 

A “high-deductible” insurance option is common among patient subscribers.  Other options are medical sharing such as Medishare and Sedera.  Health insurance can ensure financial help with higher-priced medications and labs, imaging, specialist visits and hospitalizations.

What about “Walk-in” visits?

Non patient subscribers do not have this access.  

As noted earlier, patient subscribers call or text Dr. Cook to set up a walk-in at any time.

Can I pay for one office visit at a time?

A patient/physician relationship requires a commitment by both parties for continuity of care.  Medical conditions often worsen without consistent follow-up care and evaluation.  

Dr. Cook will see only patient subscribers and members of their household.

Can I contact Dr. Cook after-hours?

As a Patient Subscriber, you will have Dr. Cook’s phone number for calls or texts.  This is one of the greatest advantages of Direct Primary Care.  If appropriate, telephone or video calls may be used in place of in-clinic encounters.  Dr. Cook will even visit you at home if necessary.
